Care management services
Medway Council's Care Management
Learning Disabilities Team provides a dedicated service for people
who have a learning disability. Care management staff will
undertake a community care assessment which will enable access
(subject to eligibility) to a range of services provided in the
home or in day or residential settings, as well as providing
information and advice. Service users can contact the team directly
and their care managers will give them their direct-dial
number.
Day care resources
A broad range of services is
co-ordinated through Chatham and Strood Opportunities Centre,
providing employment and day activities for people with a learning
disability, including ironing and catering services and supported
employment.
Day
centres offer a range of activities such as personal
development, social integration and community awareness for people
with a learning disability.
Respite services
Two small residential homes provide
respite and phased care for a range of learning disabled clients,
while their main carers have a break from caring.
People without a care manager who
would like to make a referral to the Learning Disability Team
should use the contact details below. Their request will be
recorded and passed to the Learning Disability Team. Someone from
the Team will then make contact to arrange an assessment that will
include access to day care resources.
