Medway Providers Forum

Medway Providers Forum (MPF) was re-launched in October 2009, to bring together the agencies and organisations with an interest in enhancing the provision of Social Care Services.

MPF meets four times a year and unites providers of a wide range of services that support people with adult social care needs.

Medway Council values excellent outcomes being achieved in partnership with providers. Medway has more than 90 per cent of its providers rated as Good or Excellent. In these tough times Medway Council cannot award uplifts in fees or rates that reflect RPI (Retail Prices Index) or CPI (Consumer Prices Index). However, as a demonstration of its commitment to the people working hard on the frontline of services, the council is making available a benefits package for employees of contracted services called More4you.

Transfer of care concerns

It was agreed at a multi-agency meeting on 24 May 2011, that providers of social care could use the Transfer of Care Concerns form (Word) so that it captures any issues that will then be discussed at a weekly meeting held at the hospital. Genette Laws, Service Manager for Commissioning, will attend that meeting and share any concerns that are reported on completed forms. The meeting is with partners from other parts of the health and social care economy. Please note that the form is only for Medway and Swale Health and Social Care professionals that support adults.

All completed forms must be emailed by 1pm on Thursday, so that they can be shared at the next Whole System meeting that is held on Fridays at 10:30am. You will receive feedback about the discussions of your specific case by the following Wednesday and trends will be shared quarterly at the Provider Forum.

Please submit your completed forms by email to the following address: carol.galvin@medway.gov.uk.  Please ensure that the subject heading includes the words 'TOCC Form' so that they are easily identifiable.
