Older People's Plan-
Significant progress has been made
over the past decade in developing a range of community-based
services that enable very frail or vulnerable people to be cared
for in the community.
However, the growth in Medway's
older population and a reduction in residential care spaces means
the council needs to change the way services are
provided.
That includes increasing the
involvement of older people; working more closely with
organisations and agencies responsible for providing services;
and greater emphasis on promoting the health and wellbeing of
older people.
A strategy document Positive Ageing in Medway - the
Older People Strategic Plan and Joint Commissioning Strategy for
Health and Social Care 2010-13, helps ensure that all
older people in Medway have the same access to services. It is a
joint strategy, produced by the council and NHS Medway, setting the
direction for working together with and for older people.
It sets out how Medway Council
and NHS Medway will take a more strategic and joined-up approach to
commissioning services for older people in Medway. The strategy
allows key commissioning intentions to be set out clearly and in
one place. These intentions have been developed from analysis of
local needs and crucially, the feedback and involvement of local
stakeholders, including older people themselves and their
carers.
