Shared Lives: the service process
How someone is placed with Shared Lives carers
Step 1
The first step in the process will be for your care manager to
carry out an assessment of your needs and put together a Care Plan
taking into account how you want your life to be. If you both feel
that 24-hour support living in a family home with Shared Lives
carers would be right for you, your care manager will contact the
Shared Lives Team.
Step 2
A Shared Lives social worker will then meet you to
find out more details about the things you need help
with. We will then try to match you with a suitable carer. If at
all possible, we will give you a choice of carers.
Step 3
We will then approach the carer and tell them about you and your
support needs to make sure they are able to meet them.
Step 4
We will then share information about the carer with you and your
family or carers.
Step 5
If you want to go ahead, introductions are made. We will take
you to the carer's house for a short visit, perhaps just for a
drink and a look around the house. You can take your family with
you if you would like to. If this goes well, more visits could be
arranged, including an overnight stay. These visits will help
everyone to decide if all are happy to continue with the process
and you can ask any questions you may have.
Step 6
If everyone is happy, we can plan a moving-in date.
Step 7
There are a few procedures that have to be followed before you
can move in. For example, funding will need to be agreed and you
will complete a licence agreement to rent your room. We will also
need to complete a placement agreement, which gives details of the
support you need and we will discuss the things you plan to do
during the day.
Step 8
You move into your new home.
