Age Concern Chatham
Age Concern Chatham supports older people aged 60+ living in the Chatham, Walderslade and Lordswood areas.
They offer community day activities which include trips and outings to places of special interest, personal and domestic care, footcare, and bathing for physical and mental well-being. Advisory services, signposting to specialist services and user friendly information are also available on request.
Access to services can be made by direct referral from a healthcare professional, self-referral or referral from a family member by telephone. Age Concern Chatham can offer a whole day placement which includes transport to the centre and home and a freshly prepared and cooked mid day meal on site.
Age Concern Chatham is a non-profit making charity and services are considered affordable. For families with no loved ones living close to them, they can offer that extra support required to ensure family members do not become isolated but remain socially included and a valued member of the community.
Age Concern Chatham can arrange a private and confidential visit to service users homes and welcome family members to be present. Home visits enable Age Concern Chatham the opportunity to answer all questions and to explain about the services they can provide. Their aim is to treat everyone as individuals and to enable safe and independent living and enjoyment of activities of daily living, reducing unnecessary hospital and nursing care admissions. Age Concern Chatham invite service users to be involved in all decision making around services and welcome feedback on how to improve the services and deliver programmes of interest.
As well as whole day placements, Age Concern Chatham offers a few hours spent at breakfast or lunch clubs. The Home Living Support Team can deliver personal and domestic care, as well as bathing and footcare assistants who would be happy to visit service users at home to deliver personal services.

Medway Older Peoples' Partnership (MOPP)
The Medway Older People’s Partnership (MOPP) was established in June 2002. It started initially as an informal partnership after a discussion with a group of like-minded people. They identified that there was a need for improving the lives of older people locally through bringing together older people and providers. This was developed into a formal board with an adopted constitution. The organisation is currently waiting confirmation as a registered charity.
The aim of MOPP is to share information and resources with both voluntary and statutory groups working with older people, bringing these groups together, to share and to highlight areas of need and concern. This way, resources can be put to best use, a strategic vision can be promoted and a voice given to older people in Medway. MOPP works together with older people and the organisations that represent them in Medway, to fulfil their needs and aspirations in a complete and joined up way.
