Extra curricular activities - chaperones

On receiving an application by an adult to be registered as a chaperone or matron, Medway Council's Education Welfare Service sends out a letter, together with an application form, a copy of the guidance notes and a Criminal Records Bureau (CRB) check form.

Having completed and returned the forms, an applicant should come to the address below with their birth certificate and where applicable, the following documents:

  • passport,
  • marriage certificate,
  • driving licence,
  • a recent letter or bill showing their current address,
  • three passport-size photographs.

Once they have been interviewed and all the documents examined, the disclosure form is sent off to the CRB and once clearance is obtained, a licence is issued, complete with a copy of the person’s photograph.
