Please use the form below to make a nomination for the Medway Sports Awards. 

Nominations should be based on achievements made between 1 October 2010 and 12 October 2012.
The closing date for entries is 12 October 2012. 

Please refer to the criteria for each category when making your nomination.  The judges will only judge the information contained in your nomination so make sure you follow the criteria and don't leave anything out.  If you have any statistics or examples that illustrate your nomination please include them.

If you want to make more than one nomination please make them separately.
