The due dates for your SMC2 form can be found by downloading the relevant form below.

The completed form should then be returned by email to food.training@ocelot.org.uk. This will ensure that you have been correctly charged each month.

Customer satisfaction sheets

The customer satisfaction survey for school meals service is to be completed each month, indicating how you rate the school meal service provided at your school. All completed forms should be returned along with your completed SMC2 form by email to food.training@ocelot.org.uk.
