Description

The flexible dementia outreach service is designed for people aged 55 and over and living in the Medway Area.

Referrals can be made by any Health and Social Care Professional or by yourself or family.

Aims of the dementia outreach service

The aim is to help individuals experiencing the early stages of a dementia type illness to continue to pursue their interests, take part in meaningful activities, maintain their independence and retain or develop social contacts.

Trained support workers will spend quality time with each person on a weekly or fortnightly basis, helping reduce social isolation and retain or regain links with their local community.

By allowing a person to remain at home longer, enabling them to retain daily living skills and independence, and by giving them a sense of achievement and purpose, disruptive or challenging behaviour can be alleviated.

Our ethos is to understand the person with dementia, and allow them to be themselves in a safe, non-medical and stimulating environment.

This gives their carer valuable respite time, reducing the potential for crisis intervention.

The service aims to:

  • build relationships
  • promote confidence
  • maintain independence
  • reduce isolation
  • improve daily living skills
  • encourage hobbies and interests
  • enhance well being
