Crossroads Care is a specialist voluntary organisation providing support to carers in their own homes, giving carers time to themselves.
Our service caters for those with critical and substantial care needs, including tailor-made care, where necessary, to meet the needs of each individual.
This can include short-term care following hospital discharge up to and including palliative end of life care.
Our service is available to those currently assessed by the Medway Social Care team and those who have been allocated financial funding by the Direct Payments team.
There is also the opportunity to purchase our service on a private basis. All our staff are trained to NVQ level 3 and have all undertaken first aid training.
They all have enhanced DBS checks.
We ensure that there is an individual care plan for all service users developed with the family carer and the person with the care needs.
Our trained Care Support Workers will take the place of the carer providing respite to the family carer.
All our Care staff are trained to provide basic care including support with:
- bathing
- management of continence
- dressing and or undressing
- mobility and transfers
- administration of prescribed medication
- therapeutic programmes
We can also offer specialist care including help with:
- artificial feeding
- rectal medication
- tracheotomy care – oral suctioning
- eye and ear drops
- Ileostomy, gastrostomy and colostomy care – change bags
- catheter care – changing bags, monitoring output
Our service caters for children and adults who have care needs as a result of disability, illness or age.
Our respite care service is provided through the provision of community based support workers and is available 24 hours a day, 7 days a week.
