The Walter Brice Rehabilitation Centre can provide day rehabilitation to adults that have physical disabilities and problems and who are registered with a Medway-based (GP).
New referrals are seen at home first to discuss attendance and then offered a place if appropriate. Patients attend one day each week until discharge and an agreed rehabilitation plan is developed which is kept under review by the team.
Patients are able to see the physiotherapist and occupational therapist as often as they need to. Following discharge, an effective recall system operates for those with a progressive condition to ensure patients continue to return to the centre as long as they need to, where appropriate.
A social/therapeutic programme of activities helps patients to achieve and maintain both confidence and independence.
