Transforming health and social care
We are making changes to the care system to improve the experience for patients and meet future issues. As people are living longer with more complex health issues at a time and the NHS budgets are tight, we need to do more to prevent health issues and help people stay healthy.
The Medway Model is in line with the Sustainability and Transformation Plan (STP) for Kent and Medway, which sets out how services need to change over the next five years to achieve the right care for people for decades to come. It will help to deliver the Five Year Forward View, which sets out the national vision for health and social care services.
