The following are examples of the support offered by Medway's Community Support Mental Health Outreach Team (CSMHOT).

Access to connect you to the local community

Rebuilding your confidence in using public transport so you can travel in your community.

Building social and domestic skills

We will help you build:

  • confidence
  • self-esteem
  • friendships
  • family networks
  • faith networks.

We will do this by helping you connect with people in your community. 

We also offer support and encouragement by working with you to maintain your home and daily life activities such as:

  • shopping
  • cooking
  • cleaning
  • budgeting
  • debt management.

Advocacy

We will help you when you need to get advice from other agencies. For example, we will help you contact the right agency to make sure you are claiming all the benefits you're entitled to.

These opportunities have allowed people to reach and maintain their full potential and to go on to live productive lives.

Bridging care, employment and voluntary work

We can help you find and take part in work and leisure activities. Work can be voluntary or paid employment. This helps you build your social network.

Securing a channel of communication

We can help you stay in touch with the teams you need for your care. For example, we can gather information on your progress and feed it back to other teams and agencies involved in your care.

Maintaining a healthier lifestyle

We can help you have a healthier lifestyle by encouraging you to eat a healthier diet.

We also encourage you to see your dentist, doctor and optician when needed. The aim is for you to continue to visit them independently.

We will support you to manage your own treatment and self-manage your medication so you can learn to do this on your own in the future.
