wHoo Cares

wHoo Cares aims to reduce social isolation and strengthen communities on the Hoo Peninsula.

It offers bespoke life planning including activities such as befriending support and transport to medical appointments, shopping, or local community activities. 

This service is for socially isolated residents on the Hoo Peninsula.

Eligibility criteria

To be eligible, you must be living on the Hoo Peninsula (ME3 postcode).

IMAGO (care navigation programme)

IMAGO helps improve support to patients by assisting them to better navigate through health, social care and voluntary sector support service.

This service for anyone aged 18 and older.

How to refer

Carers First 

Carers First provides information, advice and guidance and support to informal carers of someone with an illness or disability.

This service is for people who are caring for someone with an illness or disability.

Eligibility criteria

To be eligible, you must be:

  • in a caring role
  • aged 5 or over.

How to refer

Live Well Kent and Medway

A support worker will give advice and support for issues that are affecting your mental health or general wellbeing.

They will help you access a range of services provided by Live Well Kent and Medway, as well as others that are suited to your needs.

This service for anyone aged 17 and older.

Eligibility criteria

To be eligible, you must be

  • a Medway residents aged 17 and over
  • experiencing mental health needs which can be safely supported within the local community.
