Complete a short break self-assessment
To complete this assessment, you'll need to tell us:
- your details
- your relationship to the child
- about the child's needs
- how this affects you
- details of other children in the household.
You'll also need to upload either:
- a diagnosis letter from the child’s school, doctor (GP), Child or Adolescent Mental Health Support (CAMHS) worker or other relevant professional
- a copy of a Disability Living Allowance (DLA) award letter showing either the higher rate has been awarded
- a copy of Personal Independent Payment (PIP) award letter showing the higher rate has been awarded.
To complete an assessment form, you'll also need to confirm you have read and understood our terms and conditions.

What happens after
We'll consider your application against the standardised scoring system and contact you about the outcome using the details provided.
Due to the large number of applications, we're experiencing delays in the time it takes for us to complete this step. The waiting time is currently up to 12 months. We're working hard to address this and have recruited more staff to help reduce the length of time that families are waiting.
