Children and Young People with Disabilities (CYPD) Team
The Children and Young People with Disabilities (CYPD) Team is a specialist service.
The service is made up of 3 teams of social workers for children and young people aged 0 to 17. There is a fourth team which focuses on occupational therapy.
Once a young person turns 18 this service will no longer support them.
If further support is needed, then this may be provided by the Adult Social Care Transition Team.
Who can get support
The CYPD Team can support a child or young person if they have:
- a diagnosed disability
- a complex health condition.
These disabilities and conditions will likely:
- be permanent (one that is not expected to improve)
- be substantial (one that is life changing or limiting)
- be at a severe or profound level
- severely impact their day-to-day activities.
The team can also support those whose needs are less evident but may benefit from support. This is determined by a needs assessment under Section 17 of the Children Act 1989.
The team does not usually provide support to a child or young person who:
- is diagnosed as having Autistic Spectrum Conditions (ASC) without a learning disability
- is diagnosed as having Attention Deficit Hyperactivity Disorder (ADHD).
In some cases where conditions severely impact children and their families, the CYPD Team may offer support.
Further support
The CYPD Team will carry out an assessment for siblings. If they are determined to be young carers, support will be offered.
The CYPD Team may also review children under:
- child protection
- children with a child in need plan.
This is only if there are relevant concerns around:
- the family circumstances
- parenting which impacts all children.
How to get support
You can request support from the CYPD Team through the Single Point of Access (SPA). This is in line with the Multi Agency Safeguarding Hub Standard Operating Procedure.
Referrals will be accepted from all professionals involved with the child or young person, family members or self-referral from the young person themselves.

Transition into Adult Social Care
Professionals from Childrens Services can make a referral to Adult Social Care when a young person is aged 17.
If your child or young person is not known to Childrens Services, you can make the referral yourself.

Adult Social Care will complete assessments. This includes a carers assessment for informal carers in line with the Care Act 2014.
