The Medway CSMHOT offers mental health support to people who:
- have completed a Care Act assessment from the Adult Social Care team
- have mental health needs that impact on your daily living and social care needs
- meet our team's criteria
- are 18 years old or over at the point of referral
- live in Medway and pay Council Tax to Medway Council.
If all the criteria above are met, a referral could be made to us to help support you with your care and support needs that have been identified by the Care Act assessment.
We can help if you have a range of mental health issues such as dual diagnosis and physical conditions which impact on your daily life.
The service is available all year, 7 days a week, including:
- evenings
- weekends
- bank holidays.

Our aims

The aim of all the support is to use a strength-based approach, to encourage recovery and improve your wellbeing.

How we'll help support your mental health
We'll make sure you are always at the centre of what we do. Our dedicated mental health and social care professionals work with you, one-to-one, to create your individual recovery plan together.
We will support you by:
- expanding and developing your trusted support network, building bridges with you to connect you back into your local community
- signposting the right partner agencies and working with them
- helping you learn and manage your daily activities in your home and in the community, to improve independence and confidence
- focussing on your strengths
- focussing on your goals and helping you meet them
- helping you back into education, voluntary or paid work
- giving you the time, support, continuity, and consistency you need.
We also:
- help to reduce the stigma around mental health in the community
- listen to those we support, carers and local communities to make sure the service always fits the needs of its users
- have independent complaint procedures for safeguarding.
The Community Support Hub
Our community support centre is based at 147 Nelson Road, Gillingham. Our hub acts as a meeting point for you to access support from us.
We occasionally run time-limited activity sessions from the hub.
The aim is to connect you further into your local community and other local community activities in Medway. This will:
- give you choices about how to meet your needs
- maximise your independence
- improve your emotional wellbeing.
What our support looks like
We have mental health and social care professionals to:
- help you create a recovery plan
- provide one to one support
- help you connect into the community.
The support you will receive is tailored to your needs that will help you achieve your goals.
Our support focusses on 3 areas:
Reablement, short, intermediate and longer-term support
We help to provide outcome focused support as identified in the individual's support and care plan. This helps the service user to maintain and develop their independence.
Intensive support and preventative crisis
This is for service users who need a higher level of support than they would normally need.
The aim is to prevent further relapse and keep you in your home rather than hospital admission. This is for a limited time to support you through your crisis.
Reablement intervention
The CSMHOT link workers support inpatients and service users to assist in their recovery and help them with the transition back to their home.
This support can be up to 6 weeks. If longer term support is needed, this can be discussed.
The cost of our services
Support is free for up to 6 weeks for short-term reablement or preventative interventions.
If the longer-term support is needed, Medway Council has a charging policy. A request for a financial help will be reviewed by the relevant team and discussed with you.
You will receive free aftercare, known as section 117 aftercare, when you leave the hospital for services which meet a need arising from your mental health illness, if you have been:
- detained under the Mental Health Act 1983 section (3) sections (47 or 48) or sections (37 or 45A)
- placed on a community treatment order (CTO).
