Example of Disability Living Allowance (DLA) proof
You'll need to upload a photo or scanned copy of your Disability Living Allowance (DLA) award letter from the Department of Work and Pensions (DWP).
It must:
- be dated within the last 12 months
- show your name and address
- show your mobility rating.
