What happens when you turn 16
Your social worker will introduce you to a member of the 16+ team when you turn 16 or 17.
This lets you, before leaving care, get a good understanding of what will happen after you leave care.
Your personal advisor will be there to help you with everything including health needs, finances and housing.
They’ll also point you in the right direction for any other issues you might have.
