Before leaving hospital you may be offered an assessment to see if you need social care services when you return home.

You may then be offered:

  • rehabilitation programmes such as occupational therapy to help you stay in your own home
  • services at home to help you with personal care, shopping and housework
  • support to keep you active and social in the community

How to get support

If you're in hospital you'll need to ask to speak to your hospital social worker about being assessed for support after you leave. 

If you're a carer and going into hospital will leave someone without care, you should call us as soon possible on 01634 334 466 so we can make alternative arrangements. 

When you leave hospital 

If you’ve lost some of your everyday skills such as cooking, washing, cleaning or dressing through illness, you could get support to recover.
