Services for adults

We provide a range of services to meet the needs of adults with learning disabilities, supported by trained and experienced staff. These include:

These can be used by adults who are over the age of 18 years and have been referred following a community care assessment by a care management team.

Improving services

A national programme, called Transforming Care, has been set up to improve services for people with learning disabilities who display challenging behaviour, including those with a mental health condition. It aims to reduce hospital admissions and support more people in the community whenever possible.

Medway Council and Clinical Commissioning Group (CCG), in partnership with Kent County Council have produced Transforming Care Plans which outline our priorities:
